As the Health Resources and Services Administration (HRSA) prepares to pilot a 340B rebate model, questions are swirling about how it will work. The pilot marks a significant shift from the 340B program’s longstanding upfront discount structure and could expand to additional drugs in future years.
Under the pilot, manufacturers of the first 10 drugs subject to the Medicare Drug Price Negotiation Program can apply to implement a 340B rebate model. Providers would have to purchase these drugs at the higher wholesale acquisition cost (WAC) and then wait for manufacturers to issue a rebate covering the difference between WAC and the 340B price. Manufacturers have already submitted their rebate proposals, and HRSA has said it will confirm next month which plans are approved. Implementation begins Jan. 1, 2026.
