Part 3 of a 4-Part Series: Is In-House Pharmacy Capture the Overlooked Strategy to Growing Your 340B Program?

SPONSORED CONTENT

The 340B program is facing a period of unprecedented change. With the Inflation Reduction Act (IRA) and Medicare’s Maximum Fair Price (MFP) provisions set to take effect in 2026, covered entities can expect significant shifts—most notably, reduced drug reimbursement and rising 340B prices. These changes will shrink the already tight margins many provider teams rely on, placing even more pressure on 340B savings to support patient care.

At CompassRx, we’ve spoken with countless provider organizations navigating these challenges. And while many are searching for new ways to strengthen their programs, we’ve noticed a powerful but often overlooked lever for growth: in-house pharmacy capture.

The Untapped Opportunity in In-House Pharmacy Capture

For covered entities, increasing in-house pharmacy capture is one of the few strategies that can simultaneously grow system volume, revenue, and 340B savings. Organizations that have embraced this strategy aren’t just seeing better outcomes for their entities but also for their patients. Data shows patients who use in-house pharmacies see improvements in patient experience, adherence, and clinical outcomes.

But despite this, in-house capture remains a challenge. While best-in-class systems may achieve 70–80% capture rates, many covered entities struggle to break past 30%.

Three Requirements for Increasing In-House Capture Rate

So how can your team address prescription leakage and drive growth through in-house pharmacy use? Based on our work across our health system and health center partners, we’ve identified three key strategies:

1. Gather Prescription Analytics

Start by quantifying the problem. Ask your pharmacy or IT team to generate regular reports—ideally monthly or more frequently—that break down prescription capture by site, service line, and provider. This level of detail is critical to identifying where leakage is occurring and where the greatest opportunities lie. Through consistent, targeted reporting, you’re better able to prioritize interventions and track progress over time.

2. Consolidate Pharmacy Benefits

Patients often don’t recognize the benefits of using your in-house pharmacy. While they may interact with multiple staff members—such as their doctor, front desk, or care team—those conversations are usually focused on immediate clinical needs, leaving little time to explain pharmacy options. That’s why it’s important to provide a simple, centralized way for patients to learn about your pharmacy’s value, whether it’s improved outcomes through coordinated care, convenient medication delivery, or even potential cost savings for them.

3. Implement Patient Engagement

Because providers and clinical staff are often focused on immediate care needs, there’s rarely time during visits to educate patients on pharmacy benefits in detail. That’s why it’s important to have a direct way to connect with patients outside of appointments. Outreach through texts, calls, or emails gives patients a simple, convenient way to learn about your pharmacy and ask questions on their own time.

How Our AI Agents Met These Requirements and More for our Partners

At CompassRx, we’ve developed AI-powered pharmacy care navigators (i.e. agents) specifically designed to address pharmacy leakage. Our AI agents support all three critical areas: they conduct daily analytics to identify leakage, learn and organize your pharmacy’s unique benefits, and automate personalized outreach to patients on your behalf.

From a patient’s perspective, a member of your team is reaching out directly to them to check on their concerns and educate them on free benefits they can take advantage of.

Compared to generic text notifications, our AI agents’ personalized outreach has achieved 3x higher capture, significantly increasing in-house pharmacy awareness and utilization for our partners. These AI agents are also trained to escalate clinical or sensitive questions directly to your staff and operate fully within HIPAA, TCPA, and your organization’s 340B compliance policies.
